MHC firmly believes that people with a learning disability should live in homes, not hospitals. We have a proven track record of supporting individuals with complex needs to step down from high-intensity hospital services into less restrictive settings, where they can enjoy a better quality of life and safe access to the community and meaningful activities.
Making a placement
Successful placements start at the point of referral. Every effort is made to ensure that individuals are safely placed within homes appropriate to level of need. Below is a brief outline of our placement procedure:

Making a Referral
Please send referral documents to placement@mhc-uk.com
Assessment
Once an appropriate service has been identified your referral will be passed to the Registered Manager who will undertake an initial assessment to determine if they can meet need. If initial indications are positive the Registered Manager will make arrangements to conduct a more comprehensive review and meet with the individual and all relevant stakeholders.
Placement Offer
If the individual and the home are a good match and both wish to proceed, the Registered Manager will make a placement offer inclusive of a service delivery plan which clearly outlines initial outcomes and plan of care.
Placement Commences
If agreeable to all, including the individual, the transition will start, and the individual will begin living at one of MHC’s care homes.
