Working towards Autism Accreditation with the National Autistic Society

Elm

Autism Service

Service Overview

Elm is a residential home for people living with a primary diagnosis of Autism, learning disability and complex needs. The home adopts a person centred approach to improve the quality of life for individuals living at Elm.

Admission Criteria

Elm supports:

  • Males or females aged 18 yrs + who have a diagnosis of autism and a learning disability. Individuals are likely to have additional needs and may display behaviours that challenge.
  • Admission is primarily based upon an initial 12 week assessment period.
Philosophy of Care

Philosophically, we believe in understanding the needs of the individual through robust assessment in order to provide person centred support to enable individuals to lead fulfilling lives within their local community.

We do this through:

  • Person centred planning and positive behaviour support to enable individuals to maintain and develop skills leading to greater independence.
  • Providing services that represent good value for money by demonstrating measurable, effective outcomes and progression.
  • Ensuring our colleagues have opportunities for tailored training and development to meet individual needs.
  • Ensuring that compliance and audit systems support service delivery and constantly drive improvement and embed quality in everything we do.
  • We work collaboratively with a wide range of stakeholders to meet the needs of the individual’s needs.
The Support Delivered
  • Individuals living at Elm are provided with the appropriate care and support to enable them to achieve their potential by enhancing well-being, independence and quality of life. The care and support provided to each person’s individual needs and wants are determined at the initial pre-admission assessment.
  • Enhancing quality of life is important to us. By working with the person and key stakeholders we adopt a person centred approach to promote the development of both social and adaptive skills, leading towards maintaining and increasing independence and offering opportunities to achieve positive outcomes.
  • By enabling staff to understand the function of risk behaviours, this enables plans to be implemented to safely manage potential risks and to provide the least restrictive support in an inclusive and empowering way.
  • An important element of the service includes individualised activity programmes, together with maintaining family contact and social networks.
