Acrefield House
Learning Disabilities Service
Acrefield is a residential home for people living with a learning disability. Individuals are supported to enhance their living skills in readiness for more independent living.
Acrefield promotes independence and choice through person centred care and risk management strategies.
Acrefield supports:
- Males or females aged 18 yrs + who have a diagnosis of a learning disability, some of whom may also have physical disabilities.
- May have additional complex needs including autism.
- Admission is primarily based upon an initial 12 week assessment period.
Acrefield adopts active support, person centred planning (PCP) and positive behavioural support (PBS) methods to promote and empower individuals to make decisions and achieve fulfilling and rewarding lives.
By working together, we develop pathways that enable individuals to achieve positive outcomes. Likewise, we work together with families and carers, so they are always fully involved in their loved one’s care whilst at Acrefield. Risk strategies are implemented to safely manage potential risks and to provide the least restrictive support in an inclusive and empowering way.
The service offers a real sense of ‘home’ in which residents are encouraged to take an active role and have their say in day to day operation. In addition to in-house activities the home promotes community focused opportunities which offer choice, decision making and social interaction. This includes going on local outings or holidays, fishing, swimming, cinema and picnics to name a few. Individual interests and hobbies are supported and facilitated.
